How to verify a pharmacist's training
The completion of an APhA certificate training program results in a certificate of achievement, but is not a certification. If you still require a statement of verification, you will need to provide the date and location of the program that was attended, along with a written release by the client. There is a $25 administrative fee per person being verified. Once the payment has been processed, an email of verification/non-verification, along with a receipt will be provided to the 3rd party.

- NOTE: APhA uses this information to verify that we are verifying the correct individual. Your client's/employee's name may be the same/similar to other individuals in our system.
- If sending in a check, the above form must be filled out completely and legibly and mailed
- If sending a check, the email with the verification/non-verification and receipt will be sent within 48 hours from the date it was received in the lockbox.
- A working email address is required
Employers: If you are looking to verify the completion of an APhA certificate training program completed by your employees, we will not need the written release from the employee; however, we still need to have the information requested on the form above to ensure that we are verifying the correct individual.
